About the roleThe Transitional Child and
Family Health Nurse is an integral part of a multidisciplinary healthcare team, working with support from an experienced Child and
Family Health Nurse to enable the completion of a postgraduate Certificate/Diploma in Child and
Family Health Nursing. The Transitional Child and
Family Health Nurse works in partnership with parents/carers to promote the health and development of the child and the wellbeing of the family unit from the birth of their child until the age of five years.
