You’ll feel a real privilege being a trusted caregiver in our patients, their families, their carers, and our communities’ lives.Where you'll be workingThis position is based within the Northern Sydney Home Nursing Team at Brookvale Community Health Centre and delivers care to people living in the home setting.What you'll be doingThe NSLHD Nursing and Midwifery Workforce will be compassionate, competent, confident, contemporary, capable and committed to leading person-centred health care.This position plays a major clinical and care coordination role through the provision of expert clinical assessment, referral and coordination of health and community services for people with complex clinical and social determinants of health living in the community who are at risk of deteriorating health that may result in declining quality of life and a hospital admission.People of Aboriginal and/or Torres Strait Islander background encouraged to apply.
Candidates will need to meet the following criteria:
- Registered Nurse - Division 1 with Australian Health Practitioner Regulation Agency (AHPRA) with relevant post-registration qualifications and at least 3 years’ experience working in the clinical area of their specified post-graduate qualification, or such other qualifications or equivalent experience deemed appropriate for the position. e.g., Substantial and current experience in Complex Care Coordination, using advanced clinical skills in patient assessment and care planning.
- Experience in practice that promotes self-management support, behaviour change and psychosocial support and a strong understanding of the health and psychosocial issues for people living with chronic conditions and their carers.
- Ability to identify and improve opportunities to enhance continuity of care between acute hospitals, primary care providers and community-based services and a comprehensive knowledge of primary health and community-based services.
