Auscare is seeking for an experienced individual to help support Clients to remain independent and meeting their goals by utilizing the Commonwealth Home Support Programme.
The Role:
This role will be working in collaboration with another's CHSP coordinator, and responsible for the safe implementation and coordination of services that fall under the Commonwealth Home Supports Program.
This is an administrative focused role, that requires a high level of phone communication, while also having the opportunity to visit incoming clients in their homes.

Key Responsibilities include but not limited to:
- Address referrals on the My Aged Care Portal and take prompt action prioritizing the needs of our clients.
- Intake of new consumers and ensuring they meet the funding program requirements.
- Manage the allocated hours for service provision on the cycle of the funded hours
- Coordinate the delivery of efficient home modification, Allied Health services, Respite and social support utilizing CHSP funding.
- Contribute to the continues improvement process to promote effective administrative procedures to ensure quality delivery of services
- Liaising with Multidisciplinary Health Professionals with respect to scheduling of services.
- Ensure accurate data entry of service hours delivered into DEX.
- Completion of rosters and co-ordination of shift replacements as needed
Required Skills:
- Minimum of 3 years exposure and experience working with My Aged Care portal to view and process referrals
- Excellent customer service focus
- A friendly and professional individual with a positive attitude and adhere to company procedures.
- Excellent verbal and written communication skills
- Effective time management and organisational skills enabling you to effectively coordinate multiple
- Proficient with MS Office
- Ability to speak a second language will be highly regarded.
- Ability to work on your own as well as in a team

- AFP Police Check

- Tertiary Qualification/Diploma Community Services
