The opportunity
The Clinical Practice Consultant (CPC) provides clinical leadership through the facilitation and support of nurses to develop, implement and evaluate models of care and clinical services that are evidence based and patient/client centred to improve clinical care, service delivery and patient/client outcomes. The CPC is responsible for training, clinical management and individual competency based assessment of staff who support clients with complex health needs in the community.
Key responsibilities
Deliver proficient clinical care to clients and their families using evidence based practice:
- Provide expert clinical knowledge and intervention for clients with complex needs, following professional standards and a client-centered care model.
- Assess, plan, implement, and coordinate care using independent clinical decision-making.
- Monitor client health support plans to ensure quality outcomes.
- Maintain accurate clinical documentation.
- Collaborate with stakeholders for optimal client results.
- Contribute clinical expertise through policy development and evidence-based practices
About you
You have passion with purpose and are driven by excellence. Your work and support enrich everyone. In addition, you will have the following skills:
- Registration with the Nurses and Midwifery Board of Australia and holding a current practicing certificate.
- Significant experience in their designated portfolio, ideally with relevant post-graduate qualifications.
- Experience in a leadership/management role.
- Current Certificate IV in Training and Assessment is desirable.
- Possesses skills, knowledge, and expertise in a defined clinical area relevant to community nursing and the requirements of the position.
- Advanced knowledge and experience in planning and implementing nursing care consistent with the responsibilities of the position.
- Demonstrates the ability to provide clinical leadership in a defined clinical area.
- Advanced knowledge of Primary Health Care Principles, Chronic Disease, and Self-Management strategies.
- Ability to develop, implement, and coordinate training programs.
- Skills in software relevant to the area of practice.
