About the Role

• Provide basic education on a defined range of topics to individual patients, carers or groups of patients or carers regarding the process of discharging from hospital to an RACF or with NDIS supports.
• Refer to and liaise with health care providers within the immediate team, and external providers inclusive of NDIS funded services, residential aged care facilities or NGOs providing services within delegated scope of practice.
• Follow defined scope of practice under a delegation model, ensuring service quality standards, occupational health and safety policies and procedures relating to the work being undertaken in order to ensure high quality, safe services and workplaces.
• Commit to patient safety and quality in the delivery of health care by actively participating in safety and quality practices
