About the role:
We are seeking a dedicated Clinical Care Coordinator to join John Hunter Hospital, focusing on optimising Outpatient Services in Orthopaedics, Neurosurgery, Plastics, and Colorectal specialties. In this role, you'll play a critical part in improving patient access and continuity of care within our outpatient departments. Your responsibilities will include facilitating seamless transitions for patients from General Practice to specialist care, managing referrals, wait lists, and appointments efficiently. You'll implement and uphold streamlined appointment processes, utilising triage guidelines to ensure timely access to care.
Collaboration with clinical units will enable the implementation of alternative service delivery methods such as Telehealth, with a focus on evaluating their effectiveness. You’ll strengthen integration across primary care networks, acute healthcare services, John Hunter Children’s Hospital, and community partners to enhance patient care pathways. Acting as a liaison between Outpatient Services, emergency department, and inpatient wards, you’ll coordinate referrals and appointments post-discharge, ensuring comprehensive patient care. Data analysis and reporting on patient care metrics will be integral, providing regular updates and recommendations for improved outcomes. Your clinical expertise, combined with strong project management, communication, and interpersonal skills, will contribute to the development of education programs for primary healthcare providers, fostering continuous improvement in clinical practices.
This position is full time only, part time/job share will not be considered due to the complex nature of the role.
Where you'll be working:
John Hunter Hospital Outpatient Service is located in the Royal Newcastle Centre. This is one of five Clinical Care Coordinators supporting a multi-disciplinary team.
John Hunter Hospital is located within the Hunter New England Local Health District and is nestled between multiple beautiful beaches and the magnificent Hunter Valley vineyards which are just an hour’s drive away. The hospital has direct access to public transport and is close to numerous public schools and several major shopping centres.
The NSW Government has committed $835 million to redevelop the John Hunter and John Hunter Children's Hospital. The John Hunter Health and Innovation Precinct project will deliver an innovative and integrated precinct with industry-leading facilities. The project will work in collaboration with health, education, and research partners to meet the current and future needs of the greater Newcastle, Hunter New England, and Northern NSW regions.
