About the Role:
As a Clinical Care Coordinator at Westfund, you'll be at the forefront of our innovative Care Coordination Program, working closely with stakeholders to design, implement, and optimize initiatives aimed at enhancing the quality and efficiency of care delivery for our members. Your responsibilities will include:
- Program Development: Collaborate with internal teams and external partners to develop and implement a comprehensive Care Coordination Program tailored to the unique needs of our members.
- Day-to-Day Coordination: Oversee all aspects of care coordination activities, including patient assessments, care planning, referral management, and follow-up care, ensuring seamless transitions and continuity of care.
- Reporting and Evaluation: Establish robust reporting mechanisms to track program performance and outcomes, collecting and analysing data to evaluate the effectiveness of interventions and identify areas for improvement.
- Quality Assurance: Implement quality assurance processes to ensure adherence to clinical guidelines, regulatory requirements, and best practices, continuously monitoring and optimizing program workflows to enhance efficiency and efficacy.
- Stakeholder Engagement: Foster collaborative relationships with healthcare providers, community organizations, and other stakeholders to promote coordination of care and improve health outcomes for our members.
Qualifications & skills
- Current unrestricted registration with the Nursing and Midwifery Board of Australia (NMBA) as a Registered Nurse
- Minimum of a Bachelor of Science in nursing or equivalent degree from an accredited nursing program
- Demonstrated clinical experience in a healthcare setting with a preference for experience in care coordination or chronic care
- Minimum 2 years relevant industry experience in Chronic Disease management and care coordination
- Strong project management skills, with experience in program development and implementation
- Excellent analytical and problem-solving abilities
- Effective communication and interpersonal skills, with the ability to collaborate with diverse stakeholders
