An exciting opportunity currently exists for an experienced, registered clinician to join our Care team. The ideal candidate will be a client focused individual who is driven to achieve client goals and desired outcomes, while ensuring all aspects of the Aged Care Quality Standards are being met.
The Clinical Care Coordinator will work closely with our planning coordinators to evaluate and implement clinical support goals and attend to client care as required.
Responsibilities include
- Provide clinical support to the Care team, our client's and their families
- Use a systematic multiple approach to clinical decision-making
- Implement and monitor the care of clients and their carers
- Collaborate with the health care team to achieve client outcomes
- Act to enhance the professional development of self to align with professional standards
- Function in accordance with legislation and regulatory requirements
About you:
- Sound understanding of Aged Care Standards
- Understanding of, and a commitment to, person centred and strength-based practices
- Registered nurse with current APHRA registration
- Sound clinical knowledge and experience
- An innovative and dynamic approach to the provision of care
- Excellent organisational and time management skills, with a high attention to detail and ability to establish priorities
- Excellent interpersonal and communication skills – written and verbal
- Ability to follow policies and procedures and implement quality processes
- Demonstrated understanding of, and commitment to, work health and safety requirements
- Previous clinical experience in aged care or community setting and/or Catheter management and wound management experience would be highly regarded.

- Current First Aid and CPR Certificate
- Current Driver's Licence
- Working with Children Check
- NDIS Worker Screening Check
