You’ll manage a specialised caseload with complex clinical needs. You will ensure clients are informed and supported by navigating services and funding models, enabling clients to live confidently and comfortably at home. Responsibilities include conducting comprehensive assessments, creating tailored plans aligned with wellness goals, and managing client relationships and feedback, escalating issues to the management team where necessary.The successful candidate will possess extensive clinical assessment knowledge, adept at managing complex caseloads, supported by industry experience and an allied health or nursing degreeAbout YouYou will have a Qualification in Allied Health or Nursing and recent clinical experience working in a related areaYou have a deep commitment in providing compassionate care to our elder clients.You are sensitive to the needs of older people of different social, economic and culture backgrounds.You are able to identify when a client’s needs have changed to ensure they continue to receive appropriate care.You are kind, even when a bit stressed out, dynamic and energetic.
