This role is for HomeMade - a related entity of Mable

HomeMade is a related entity to Mable and operate as a completely separate business, however both have a shared vision for self-management. 

We are a health-tech start-up established in 2020, focusing on transforming home care in Australia through our creative approach which is designed to support people in achieving their goals whilst living at home.

Leveraging on our technology platform, we are an approved provider that specialise in self-management. We partner with our customers to create unique support plans that meet their needs and optimise their funds, making the most out of their Home Care Package (HCP). Our approach is simple and keeps people in control of their package, funding, and support.

Role responsibilities:

Effectively deliver on all requirements of the Clinical Roster including:

  • Manage the clinical Inbox and task list
  • Triage the customer health and wellbeing notes for identification of, and response to, deterioration 
  • Completion of Validated Assessment Tools (VATs)
  • Assist the Support Partners via booked sessions to deliver clinical guidance and advice
  • Complete referrals for Comprehensive Nursing Assessments (CNAs)
  • Complete clinical reviews, formulating recommendations to promote health, safety and wellbeing for our customers, and supporting customers and their representatives to understand the recommendations and to make informed decisions to accept or decline recommendations
  • Review and respond to clinical escalations, and to incidents and complaints with a clinical component
  • Provide guidance and support to customers and their representatives on palliative care and advance care planning
  • Provide subject matter expertise and guidance on HomeMade clinical policies and procedures to contracted service providers
  • Uphold delivery of services in accordance with the Aged Care Act and Aged Care Quality Standards 

This role will also see you:

  • Participate in, contribute to and implement clinical quality improvement and clinical risk management initiatives, as allocated by the Clinical Team Leader. 
  • As directed by the Clinical Team Leader, provide clinical subject matter expertise and support to wider business
  • Maintain practice alignment with the Clinical Governance Framework at all times
  • Maintain AHPRA registration and CPD requirements
  • Work with the Clinical Team Leader to recognise opportunities for service innovation and take a proactive approach to implementation. 
  • Promote a culture of best practice and shared knowledge by sharing your deep experience in the home care sector and the broader aged care system.

Your skills and experience:

  • Tertiary qualifications
  • Demonstrated experience in Home and Community Care, ideally with Aged care experience 
  • Well-developed understanding and delivery of clinical support and case management experience in Aged care or Disability 
  • Current First Aid Certificate
  • Current Australian Full Drivers Licence and access to your own comprehensively insured vehicle
  • Strong communication  skills and customer service focus
  • Knowledge of Health or Community Aged Care Services or Home Care (desirable)

This role is a full time position working 08:30-17:00 Monday-Friday.
