• Salary: $84,870 - $107,279 p.a (pro rata) - RN/M2C
• Permanent Part Time position working 22.8 hours per week

OverviewThe Clinical Nurse (Cardiac Respiratory Inpatients & Home Oxygen) provides nursing services in the Adelaide Hills Community Nursing team.The Clinical nurse, Cardiac and Respiratory is responsible for:The provision of a high level of support and work in the development and implementation of effective and efficient respiratory and cardiac services aiming to:

• reduce emergency department presentations and hospital admissions for clients with respiratory and cardiac conditions
• reduce length of stay for patients with respiratory and cardiac illness admitted to BHFLHN hospitals
• timely transfer of respiratory and cardiac patients from Metropolitan health services to their community or residence
• Facilitate effective use of health services by vulnerable groups
• Supporting the education, self management and rehabilitation of participants to best manage their health condition
• Delivery of SA Health Home Oxygen Program, including Assessment, care planning , evaluation and review.
