About Us:
Albury Wodonga Health is one of the largest Regional health Services in New South Wales and Victoria, with a unique cross border health service that provides excellence in care to our regional communities. We are a dynamic organisation undergoing significant growth- with a new Emergency Department planned for 2024. The combined presentations between our two ED’s is approximately 70,000 per annum, with a variety of presentations of all aged groups and specialties.

Our Aged Services Emergency Team (ASET) Service
ASET is based in the Emergency Departments (ED) of AWH. ASET Care Coordinators are responsible for providing expert, appropriate and coordinated screening, comprehensive assessment, and care planning of people over 60 years of age, with the aim of preventing otherwise avoidable re-admissions to AWH and re-presentations to EDs and contributing to the effective flow of patients through E.Ds.
ASET works with other AWH and non-AWH services to provide clinical / professional leadership in advocating for the care of older people, the development of appropriate clinical pathways, protocols, policy, and models of care, which are informed by evidence-based practice and local clinical research. With the expansion of the Albury Emergency Department in 2024, the ASET service will be extending their operational impact, while maintaining an ongoing service across both Emergency Departments.

Who are you?
- You are committed to providing the highest quality patient centred care.
- You hold speciality qualifications in Gerontology/Aged/Emergency Care or looking for a supportive development pathway to achieve this.
- You have recent experience (5 years minimum) in Acute, Emergency OR Critical Care Nursing or Allied Health.
- You possess the ability to work autonomously and as a team member within an inter- disciplinary environment.
- You have demonstrated experience facilitating and coordinating complex patient needs.
