Clinical Nurse Consultant - Hospital Aged Care Liaison Team (Halt) (527741)

The role:

• Provide expert clinical advice and leadership within an interdisciplinary team providing assessment, care coordination and implementation of planned outreach support to older people in the emergency department, community or residential Aged Care setting to enable care needs to be met in the most appropriate environment.
• Undertake a broad consultative role by initiating and maintaining a comprehensive care network, building and maintaining links with providers in the primary health setting, including community service providers, General Practitioners, Residential Aged Care Facilities and the Rural Inpatient Facilities.
• Develop and implement evidence based practices that promote the efficient and effective provision of patient centred Aged Care services within the healthcare setting.

Details of Appointment

Fixed term part time, shift-worker position working 53.2 hours per fortnight, commencing from as soon as possible until 28 September 2024.

Salary: $111,506 - $118,336 per annum, pro rata. Our Employer 11% superannuation contribution is on top of this amount.

You’ll have access to salary package a range of benefits including living expenses up to $9,010 + $2,650 in meal entertainment each FBT year + other benefits if eligible.

• Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023 and Nurses and Midwives (Tasmanian State Service) Interim Agreement 2013

1. Registered with the Nursing and Midwifery Board of Australia

This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered.

1. Holds or is working towards a relevant post graduate tertiary qualification

2. Current Driver’s Licence
