About the Role
You will be responsible for delivering general and nursing-specific clinical assessment, management, follow-up and care-coordination in a multidisciplinary team to enable the group-based and individualised rehabilitation of people with medical and surgical cardiopulmonary conditions after recent hospital admission.
You will be working in a dynamic environment as the team's model of care shifts to flexible options and hybrid modes of delivery for clients while maintaining cardiopulmonary clinical best-practice guidelines.
Availability to work rostered Wednesdays and Fridays is essential.
Classifications for this position will range from:
Clinical Nurse Consultant Grade 4A ZF4 ($107,962 per annum, pro rata)
Clinical Nurse Consultant Grade 4B ZJ4 ($113,646 per annum, pro rata).
Your Contribution
Current nursing registration with AHPRA
Post-graduate qualifications in Cardiac, Pulmonary or Rehabilitation clinical areas, or in psychosocial areas of motivation and behaviour modification
Outstanding specialist clinical assessments, advice, education, management of people with cardiopulmonary conditions, in liaison and teamwork with other disciplines
Proficient interventions in group or individualised settings; in telehealth, telephone or in-person modes
Co-ordinate, develop and deliver the education program
IT and telehealth skills
Input into refining team model of care and other quality activities in a dynamic environment
Current National criminal history check, or willing to obtain
