ABOUT THE ROLE
The purpose of this role is to provide clinical expertise and consultancy through advanced clinical leadership, operational direction, and advocacy relevant to the speciality of Aged Care and Gerontology. The Clinical Nurse Consultant (CNC) provides direct and indirect care as required and works in collaboration with the
Nurse Navigators throughout the Central West Hospital and Health Service. The Clinical Nurse Consultant (CNC) will support the provision of safe quality healthcare through exemplary clinical nursing standards of practice for the specialty of Aged Care and Gerontology. The role maintains a clinical workload providing clinical decision direction utilizing contemporary gerontology management principles with thorough effective planning, facilitation, coordination, and evaluation of service provision.
SKILLS & EXPERIENCE
· Demonstrated knowledge and skills to be able to provide direct client care at an advanced level in accordance with contemporary nursing practice in a rural/remote community setting.
· Demonstrated high level knowledge of the Aged Care Sector and Aged Care Reforms
· Demonstrated ability to be able to provide individual Gerontology management in collaboration with client utilizing clinical pathways.
· Demonstrated advanced communication skills with individual clients, family members and other health professionals to deliver best health outcomes.
· Demonstrated ability to apply knowledge of best practice continuous improvement processes and research activities to practice.
· Understanding of and commitment to Queensland Health’s core values of caring for people, leadership, respect and integrity.
· Demonstrated high level knowledge in technology and delivery of service provision via Telehealth
MANDATORY REQUIREMENTS
· Appointment to this position requires proof of qualification and registration with the Nursing and Midwifery Board of Australia (NMBA) as a Registered Nurse (RN). Certified copies of the required information must be provided to the appropriate supervisor/manager prior to the commencement of employment.
· Minimum five years post graduate experience
Highly desirable
· Documented experience within the Aged Care Sector and Gerontology spanning two years.
