Working out of the Logan Hospital you will be a part of a multidisciplinary team that provides a 24-hour acute mental health assessment service for consumers presenting to the emergency department (ED) with acute mental health concerns.

About the Role
- The Clinical Nurse Consultant (CNC) Care Coordinator (CC) provides direct and indirect care as required and works in collaboration with the Team Leader, Nurse Educator and wider emergency department and mental health care teams to support the clinical unit in provision of safe quality healthcare through exemplary clinical nursing standards of practice for the specialty of mental health.
- takes a leadership role in liaising with stakeholders in the ED and across community mental health teams including the Transitions Coordinator to assist in streamlining consumer flow across various mental health settings including ED, community and inpatient services through proactive care planning.
- works collaboratively with consumers and stakeholders on acute management plans, safety plans and relapse prevention plans to streamline assessment and intervention for consumers with complex mental health concerns presenting to the ED.
