The Clinical Nurse Consultant is a registered nurse who practices at an advanced level of clinical skill and demonstrates specialised clinical expertise in Coordination.

About the role
The purpose of this role is to provide a central point of coordination to work towards broader systemic solutions for sub-acute long-stay patients with complex clinical needs requiring professional leadership, advocacy, advice and consultation to support and lead multidisciplinary teams to access appropriate and necessary disability or aged care supports, as a part of supporting the continuum of care from hospital to community.

About you
Our ideal candidate will be someone who can demonstrate the following:
- Have a minimum of 5 years acute nursing experience, and experience in advanced clinical care, coordination and team leader roles.
- Demonstrated or ability to acquire knowledge of flow between internal Health Service rural facilities and external facilities
- Demonstrated advanced level clinical knowledge, expertise and skills in managing complex long-stay patients.
- Demonstrated ability to work autonomously and provide independent professional judgement and expertise in problem solving and clinical management within a complex, fast paced clinical environment
- Proven ability in the application of highly developed interpersonal, problem solving, negotiation and communication skills, to build sustainable relationships with key stakeholders inclusive of Multi-disciplinary Teams.
