Job description

Please note this position is also being advertised as a Allied Health Professional - Continuing Care Team (Extended Hours) (514805d) and only one position will be filled on merit. Please note this position is eligible for a Multi Disciplinary Allowance.

The role:

The Clinical Nurse SpecialistContinuing Care TeamExtended Hours:

  • Works within the designated region in collaboration with other staff within the Continuing Care Teams, the Acute Mental Health Stream, Alcohol and Drug Services and a range of community service providers, to deliver improved outcomes for clients of the Continuing Care Stream and their families/carers.
  • Works as part of a multidisciplinary team to deliver a seven (7) day week, Extended Hours service including the provision of assertive case management/care co-ordination to a designated number of clients, participation in a duty officer roster as required, and after-hours triage and crisis support for both a designated Continuing Care Team and, when rostered, the broader Continuing Care Stream.
  • Is a clinical expert who acts as a clinical resource for staff and other providers specific to the management of individuals with serious and enduring mental health illness as required.

Duties: 

  • Provide discipline specific clinical expertise, supportive advice and clinical care across the broader community, based on best practice principles and legal and professional requirements, using specialist assessment and screening tools to deliver improved outcomes for clients of the Continuing Care Stream and their families/carers.
  • Provide treatment recommendations and specialist support to the multidisciplinary team, other health professionals and community support organisations as required and assist with clinical decision making and the planning, implementation and evaluation of care given to Continuing Care Stream clients to facilitate a person- centred and recovery focused approach.
  • Provide support and psychoeducation as appropriate and involve consumers and their families/significant others in the identification of their needs, formulation of goals, development of care plans and the implementation of care.

Details of Appointment

Permanent part time, shift-worker position working 38 hours per fortnight, commencing as soon as possible. 

*notwithstanding hours may be negotiated with the successful applicant

  • Applicants should note that, for a period of twelve months from the date of publication, this selection process may be used to fill subsequent or similar full time and part time vacancies.

Salary: $103,466 - $109,165 pro rata, per annum. Our Employer 11% superannuation contribution is on top of this amount.

  • Salary range is in accordance with Nurses and Midwives (Tasmanian State Service) Agreement 2023

Eligibility:

Successful applicants will be required to meet the essential criteria:

  1. Registered with the Nursing & Midwifery Board of Australia as a Registered Nurse and possess specialist tertiary graduate or Post graduate mental health/psychiatric nursing qualifications; or completed, prior to the transfer of nurse education to the tertiary sector, an accredited hospital-based program that lead to registration as a psychiatric nurse by the relevant nurse regulatory authority within Australia, New Zealand, Canada or the United Kingdom.
  2. Current Driver’s Licence.

*Registration/licences that are essential requirements of this role must remain current and valid at all times whilst employed in this role and the status of these may be checked at any time during employment. It is the employee’s responsibility to ensure that registration/licences remain current and to advise the Employer if their circumstances change. This includes notifying the Employer if a registration/licence is revoked, cancelled or has its conditions altered.

Applicants should note the following criteria are desirable:

  1. Extensive relevant psychiatric nursing experience and/or credentialed as a Mental Health Nurse by the Australian College of Mental Health Nurses or equivalent.
  2. Holds or is working towards relevant tertiary qualifications.
  3. Five or more years’ post graduate clinical nursing experience.

The Head of the State Service has determined that the person nominated for this job is to satisfy a pre‑employment check before taking up the appointment, on promotion or transfer. The following checks are to be conducted:

  1. Conviction checks in the following areas:
  1. crimes of violence
  2. sex related offences
  3. serious drug offences
  4. crimes involving dishonesty
  5. serious traffic offences 
  1. Identification check
  2. Disciplinary action in previous employment check.
