The Clinical Nurse Specialist Grade 2 in Palliative Care is responsible for delivering complex specialist nursing care and leading the development of specialist clinical practice and service delivery to provide effective quality care for patients/ clients with complex care needs, relating to a life limiting illness, including the provision of care and support to families and carers of these patients.
This role will require specific skills and knowledge in palliative care, to allow the provision of a client centred, consultative service for people with advanced life limiting illness, their families and carers. The provision of this care will occur across a range of settings, including inpatient, home and residential aged care. As a telehealth enable LHD this role will provide care through a range on modalities including face to face, telephone and telehealth.

Where you’ll be working:
The District wide - Specialist Palliative Care Service, includes a team of specialist nurses, a Medical Officer, Social Workers Occupational Therapists and a Dietitian. This team is supported by Clinical Nurse Consultants in Palliative Care.
Together this team work closely with other care providers and services to support clients, their families and carers across the regions of the health District.
This position will be required to provide specialist palliative care consultation services in Parkes, Forbes and surrounding communities of the Local Health District, via a range of modalities.
The Specialist Palliative Care Team is a consultation service team, providing care in the community, residential aged care facilities and inpatient hospital settings.
The Specialist Palliative Care Service of the Western NSW Local Health District is a dedicated multidisciplinary team, supporting patients with a life limiting illness across a number of care settings including hospital, community and residential aged care facilities.
