  • Salary: $84,870 - $107,279 p.a. (pro rata) – RN2C - Temporary Part-Time – working 32 hours per week, up to 1 February 2025 

About the Role:

An exciting opportunity is presented to work within the Northern Adelaide Local Health Network as a Clinical Nurse within the Aged Care, Rehabilitation and Palliative Care Division.

We are seeking an enthusiastic, passionate and energetic Clinical Nurse to join our Transition Care Program (TCP). The Transition Care Program is an established Commonwealth funded specialist subacute program. The program delivers short term care that seeks to optimise the function and independence of older people following a hospital stay. Transition care is goal orientated, time limited and therapy focused. The program provides older people with packages of services that include low intensity therapy such as physiotherapy, occupational therapy as well as social work, nursing support or personal care to maintain and improve physical and or cognitive functioning.

The NALHN TCP includes both residential (within a Residential Aged Care Facility (RACF)) and community tranches to support exit from hospital and to avoid premature entry into residential care.

As a Clinical Nurse you will provide nursing and/or midwifery services in a variety of health service settings which has been consolidated by experience and/or further study, developing from a competent to proficient practitioner. In this role you will accept accountability for your own practice standards, activities delegated to others and the guidance and development of less experienced staff. The Clinical Nurse role continues to be predominantly clinical in nature, however employees are assigned appropriate portfolios.

The autonomous role of Clinical Nurse provides opportunity for further learning and upskilling into other senior nursing roles such as Geriatric Nurse Consultant. Successful applicants may also have the opportunity to experience working in new community based Geriatric Services that are due to launch within NALHN later in 2024.

To be eligible for this position, you must be registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and hold, or be eligible to hold, a current practicing certificate.
