About the role
The Transition Care Program (TCP) is responsible for the delivery of services to meet the needs of older people with significant needs as they exit hospital. The Transition Care Program supports older people in their transition from the hospital into appropriate community support, health, and rehabilitation services.
The CN/AHP will assist the Clinical Unit Manager in the planning, coordinating and acceptance of clients to the Transition Care Program.
The TCP CN/AHP Coordinator is required to:
- Liaise with hospitals to identify suitable older people within CALHN inpatient services; ensure that those older people who are identified to be part of the program are discharged to appropriate rehabilitation services; develop short-term care coordination plans for those patients assessed; ensure ongoing case management is established and provided in the community following the patients discharge from the program; and ensure that appropriate service linkages are maintained, particularly across the aged care agencies, primary health care and General Practice.
- Accepts accountability for their own practice standards, activities delegated to others and the guidance and development of less experienced staff.
- Facilitate Aged Care Assessments as required for clients referred to TCP to ensure safe and appropriate transfer of people to TCP or ACAT dependent services and to enable key performance indicators to be achieved.
Qualifications
- Registered or eligible for registration as a Nurse with the Nursing and Midwifery Board of Australia and who holds, or who is eligible to hold, a current practicing certificate.

- Aged Care Screening check
