The Bolton Clarke Homeless Persons Program actively seeks to engage and offer a holistic primary health care response to individuals and families who are at risk of or who are experiencing homelessness. The program is committed to offering practical health care at the point of contact and actively works with people to link them into generalist and specialist services.
We’re looking for a Community Care Aide to support clients who are homeless or at risk of homelessness in metropolitan Melbourne. This role is permanent full time 5 days a week Monday to Friday. The purpose of this role is to assist clients in daily activities as determined by the Community Health Nurse and Client, and to ensure the client environment is safe, clean, and comfortable.
Responsibilities:
Actively seek and maintain supportive and therapeutic relationships with clients
Assist clients with a broad range of daily activities whilst maintaining a safe environment
To assist clients in social integration activities that enhance quality of life
Develop and maintain collaborative relationships with staff based at the SRS and with other health services (i.e. GPs and allied health services)
Ensure professional boundaries and client confidentiality are always maintained
Accurately complete client documentation
Report any changes in client condition to the Community Health Nurse
Participate in planning and managing daily workload with the Community Health Nurse
Transporting and supporting clients to attend to medical and allied health appointments as required
Communicate outcome of appointments to the Community Health Nurse or SRS as appropriate
What we are looking for:
Current First Aid Certificate & CPR desirable
CHCCS305A - Assist clients with medications (or willingness to obtain)
Certificate III or IV Community Services (Aged Care), or Certificate III in Home & Community Care
Awareness and understanding cultural and social diversity, in particular people marginalised by homelessness and/or misuse of substances
Excellent client engagement skills
Ability to provide person centered care
Ability to work in a range of environments and settings
Knowledge of, and ability to implement standard precautions when providing care
Excellent communication skills both written and verbal
Ability to contribute to and, work as part of a dynamic team
Excellent organisational and time management skills
