About the Role
We are seeking Community Care Workers to carry out a range of Community Care support tasks as described in client’s care plans by supporting and enabling the clients with domestic tasks, general health and personal care needs, assistance with community access, promoting independence, delivery of community meals, and ensuring that each client receives a high level of care and support.
Key Responsibilities
- Undertake basic domestic assistance
- Provide assistance with personal care and respite care
- Undertake the delivery of meals when required to do so
- Assist with the delivery of a range of social inclusion programs
- Monitor the wellbeing and status of clients and identify and report any potential or actual client risks or concerns via established procedures
- Provide assistance with basic ad-hoc tasks
- Assist with other services within ageing and inclusion as required
Qualifications and Experience
- MUST HAVE Child Protection Certificate
- MUST HAVE Current Level 2 First Aid Certificate
- MUST HAVE Current Drivers Licence
- MUST HAVE Current Covid-19 Double Vaccination Confirmation
- MUST HAVE Current National Police Check
- Previous Aged & Disability care experience
- Excellent communication skills with the ability to work effectively within a diverse community often with clients from different cultural and/or linguistic backgrounds
- Good decision making skills
- Capacity to work independently and manage own time effectively
- Demonstrated understanding of the issues faced by older people, people with disabilities and their carers
