About The Role
We are looking for Registered Nurses to work as Care Coordinators to support the most vulnerable people in our Community. We support people in the self-management of their health conditions including Chronic Heart failure, COPD, asthma and complex biopsychosocial needs. This role is integral in the assessment and management of client's wellbeing, and operates from a person - centred care framework aligning with the social model of health. With a high level of attention to detail, have exceptional clinical expertise, knowledge and skill
About You:
You are a dynamic Registered Nurse. With a genuine passion and interest in this unique opportunity
We work mainly Monday to Friday with some coverage required on weekends and public holidays
As you will work across various Monash Health sites, and home visiting a valid driver’s licence is essential.

Established escalation pathway to Nurse Practitioners and Medical Professionals when required
Completing all mandatory and competency assessment training

What you Need
Essential:
Current AHPRA registration
Demonstrated recent experience as a registered nurse.
Valid Driver's licence
Desirable:
Previous Community Nursing experience
Post-graduate qualifications
