About the facility
Barwon Health Palliative Care provides Palliative Care nursing, medical, allied health and volunteer services within a framework of integrated health service involving close collaboration between community, inpatient as well as an acute consultative service. The model of Palliative Care is based on the level of need and targets patients with complex care needs. The Community Palliative Care team are based at the McKellar Centre.
About the role
This role you will provide clinical Palliative Care nursing to people who have complex care needs as a result of experiencing a life limiting illness for whom the primary treatment goal is quality of life. This care is provided within an interdisciplinary team context, and in community settings. Clinical leadership, education and support is provided to Palliative Care colleagues, and generalist staff who work with a palliative approach.
