Belong at Home (CHC) is seeking enthusiastic, motivated and caring people to join our South West team as a Community Nurse.
We have casual positions available for a Registered Nurse with a highly respected not-for-profit community organisation providing care and support services across the South West of WA for Bunbury, Collie, Donnybrook, Busselton or Margaret River due to increasing demand for services in these area's.
As a Registered Nurse (Community) you will:
- Undertake clinical assessments of home care clients
- Providing excellent care and service to clients in line with their personalised care plans
- Monitoring chronic diseases and palliative conditions
- Wound care, continence care, palliative care, medication management and management of chronic diseases.
- Achieving the optimum care for clients by being responsive to their changing needs
- Liaising with and referring to other organisations as required to provide for the clients needs
- Assist the clinical leads with other duties as required.
Essential:
- Current AHPRA registration
- Availability to work on a casual roster (M-F) - school hours available.
- Aged Care experience is an advantage.
- Excellent verbal and written communication skills
- A genuine enthusiasm, care and compassion in your commitment to working with the elderly
- A current “C/CA” class WA Driver’s License is essential
- A National Police Clearance is required
- Influenza Vaccinations required (exemptions considered)
