About the role
We are looking for a Community Registered Nurse to provide support to customers living in Perth Metro area and surrounding areas. This rewarding role will see you utilising your nursing skills, whilst working within a friendly, supportive team. The role willhave you working as part of the Avivo nurse team, providing home visits when needed, training and teaching external agencies when requested, and a telephone on call service once per month.
Duties include
- Attend clinical appointments as required about the customers current and future care needs
- Monitor changes to customer needs and provide feedback for required and suggested changes to customer to the nurse team and coordinator.
- Education and training of support workers through a buddies system to demonstrate and explain care delivery for the customer.
- Liaising with families and coordinators around clinical care needs for customers.
- Attending and contribute in regular team meetings.
- Some regional travel may be required
Selection criteria
- Current APRHA registration (essential)
- Current Senior First Aid and CPR certificates or willingness to obtain one (the successful candidate can obtain through Avivo)
- Minimum 5 years of experience
- Experience supporting and developing staff, preferably in a community setting is desirable
- Experience working with children, adults, disabilities and/or older people
- Evidence of good computer literacy
- Excellent communication skills including verbal, written and listening
- Current NDIS Worker Screening check or willingness to obtain one
