About ECHO
ECHO Community Services is a registered charity providing care to the elderly to help them live in their own home with dignity while still maintaining their independence.
We provide in-home services 7 days per week for Home Care Package and 5 days per week for CHSP.
Our goal is to provide services at the same time, on the same day and with the same team of Support Workers which means that you know the people who are coming and the time they will arrive. Having the same support workers providing services, in particular, medication prompts and personal care, maintains the person’s dignity and well-being. You know and can trust the people attending. There is continuity of care for people who receive our services.
Your duties will include but not be limited to
- Ensuring full compliance with and maintenance of aged care standards.
- Ensuring that all clinical and other care needs are delivered in a safe and respectful manner that promotes the personal values of the client.
- Adhering to relevant legislation that governs quality standards, professional clinical registration requirements and standards of practice, including mandatory reporting responsibilities; this includes reporting and documenting any concerns where the well-being of a client may be affected.
- Assisting clients to meet their clinical and other care needs in a manner that promotes independence and choice in accordance with their individualised Care Plan.
- Accepting responsibility for specialist clinical portfolios including, but not limited to: continence, nutrition/weight management, palliative care, wound care, falls management, behaviours of concern, dementia, medication management, pain management, infection control, catheter and stoma care
- Undertaking data analysis in respect to client care.
- Application of clinical care indicators to client wellbeing.
Desired Skills and Experience
Essential:
- Post-registration experience working in the residential Aged Care sector.
- Registration with AHPRA as a registered Nurse.
- Full vaccination against COVID-19 and Flu vaccination.
- A valid First Aid Certificate (Ongoing Renewal required).
- A satisfactory National Police Clearance Certificate obtained within the last 6 months.
- Have reliable car with the willingness and confidence to drive in the metro area.
Competency:
- Strong person-centered skills and behaviours in communicating and engaging with clients with dementia and/or high clinical/other support needs.
- Demonstrated ability to successfully assess, plan, implement and evaluate client care in consultation with a multidisciplinary team.
- Strong analytical and problem-solving skills, with the ability to organize and prioritize tasks, work effectively under pressure, meet deadlines, and use resources effectively, safely, and efficiently.
- Demonstrated knowledge and understanding of continuous quality improvement, quality standards, and the accreditation processes.
- Have excellent interpersonal skills and the ability to communicate with people from diverse backgrounds and experiences.
Desirable
- Qualification in Dementia care and behaviour support or significant experience
- Qualifications in Palliative Care
Standard hours are between 8:00 am - 4:00 pm Mon-Fri, however, you must be willing to participate in a flexible roster involving extended hours.
