Community Registered Nurse

We are seeking a passionate and dedicated Registered Nurse to join our fast-growing Community team. Reporting directly to the Community Manager this is an integral leadership role with ongoing opportunities for growth and development.
The role will be responsible for:
- Initial and ongoing assessment and care planning of all clients
- Developing and maintaining relationships with key external stakeholders such as Local GP’s and a wide range of services offer from the south west health campuses
- Work as part of a multi-disciplinary team that is committed meeting client goals, prevention and pro activity in all clinical care.
- Maintaining care plans and providing advice to our front-line care staff.
- Management of clinical care needs of all clients
- Assessment and Care planning for chronic health conditions in liaison with GP and specialty services
About You – Key Requirements to be successful in this role:
- Current registration with AHPRA as a Registered Nurse.
- Previous experience within community services, aged care (highly desirable).
- Ability to lead a team.
- Sound understanding of people living with dementia and/or people living with complex care needs.
- Passion for working with the elderly.
- Commitment to a person-centred approach to care interventions.
- Sound clinical assessment and documentation skills.
- Well-developed interpersonal and communication skills.
- Excellent relationship building and influencing skills.
- Ability to work independently.
