AddressWe are on the lookout for several Registered Nurses and Case Managers for a small community provider located across Sydney Metro. You will play a vital role in enhancing client management, ensuring their satisfaction, and promoting cost-effective service delivery.
You will have flexibility over your caseload with the option of hybrid working, remotely from Home for up to 3 days/week.
- This is a niche provider, providing services to a Chinese and Asian Community.
- You will be responsible for managing a portfolio of community based elderly clients and assisting with their home care packages.
- You will asses their needs, develop personalised care plans and ensure the clients are getting the best possible care out of their funding.
- This provider covers Sydney Metro, Hornsby, Chatswood and other locations.
Key Responsibilities:
- Build lasting relationships with clients, care workers, and health care professionals.
- Collaborate with the operations team to support clients in achieving independence, well being, and peace of mind.
- Assist clients in understanding aged care funding and navigate the funding system.

- Competitive Compensation: Expect a competitive salary ranging from $67,000 to $75,000, with experience determining the final figure.
Skills:
- Essential: Nurse with AHPRA Registration.
- Desirable: Qualification in Community - Certificate 4 in Aged Care and/or Home and Community Services, Care management, or Nursing.
- MUST speak either Cantonese OR Mandarin.
