A bit about the role (Hazel Home Care)
Reporting to the Service Manager the Registered Nurse (RN) role is responsible for ensuring safe and quality clinical nursing care is delivered in a customer-centric way to enable more older Australians to remain living at home safely.
Key responsibilities
- Undertake comprehensive review and assessments in collaboration with customer
- Wound care and drain management
- Medication administration
- Blood sugar monitoring and diabetes education
- Liaising with other health professionals such as medical teams, pharmacists, allied health professionals and other specialists
- Documentation, reporting and the updating of care plans
- Falls management, identifying and mitigating risk
- Ensure strict confidentiality is maintained in relation to customer
- Palliative care and family support
Key selection criteria
• Registered Nurse (RN) with Community or Care Management experience
• Excellent clinical knowledge and experience
• Empathy and understanding of older Australians particularly those living with dementia or a disability
• Highly professional and able to act with compassion
• Exceptional time management skills
• Excellent administrative and computer skills
• Excellent communication skills
Requirements
• Registered Nurse (RN Div 1) with AHPRA registration
• Social, Home & Community Services, Aged Care, Disability or related relevant experience
• Current driver's licence, own car, car insurance and safe driving record
• Own car & willingness to travel to visit customers
• Federal Police Check
