Job Title: Community Support Worker

Location: Home & Community - North, Southern Cross Care Tasmania

Job Description:The role of the Community Support Worker is to provide in-home support services to clients which actively promotes their independence and community participation, all whilst maintaining a safe working environment and the adoption of safe work practices.

Responsibilities:

1. Ensuring the highest standard of care is delivered to care recipients

2. Assisting in the implementation of support described in support plans

3. Undertaking support tasks in accordance with the clients individual support plan

4. Ensure work practices are continually improved and comply with the Aged care Standards

5. Maintain good relationships with the care recipient and families 

6. Reporting and documenting all issues/concerns/matters in accordance with internal policies and procedures

7. Take into account an individual's cultural and religious preference when interacting and assisting in support activities. 

Required Qualifications & Skills:

1. Certificate 3 level Community Services and/or other relevant courses.

2. Previous experience working in a community-based role would be advantageous.

3. Excellent interpersonal and communication skills.

4. Physical ability to support clients with their mobility.

5. Genuine passion for working with the elderly and building a sense of community.

6. Ability to maintain a high level of professionalism and adhere to all privacy guidelines and regulatory standards for aged care.

7. Current driver’s license and access to a reliable vehicle.

8. Current police check, or the ability to obtain one.

9. Tasmanian Working with Vulnerable People registration with NDIS endorsement (or able to get one)
