About the Role:
The Acute Allied Health Service is based within the Division of Allied Health and provides Allied Health services and interventions to a range of inpatient and outpatient settings at the Canberra Hospital. The Acute Allied Health Services consists of the following departments: Aboriginal and Torres Strait Islander Liaison service, Psychology, Social Work, Speech Pathology, Audiology, Occupational Therapy, Physiotherapy, Nutrition and Dietetics and Exercise Physiology. The Acute Allied health services provide an on call and after-hours service including weekends.
Under the direction of the Director of Allied Health, the Complex Care Coordinator is responsible for coordinating Complex Care needs and discharge planning leadership for General Medicine patients in the multidisciplinary team. General Medicine works with a range of patients with medical conditions, many whom have complex medical and social needs.
This role will work closely with the allied health team, unit CNC and medical team to problem solve discharge barriers, coordinate patient care with the treating team and advocate to external agencies to facilitate safe and timely discharge.
This role will coordinate the Allied Health team on the unit and participate in activities to represent Allied Health on the unit. This role involves liaising across Canberra Health Services, partaking in service improvement and quality assurance initiatives and contributing to the supervision and training of fellow Health Professionals and Students. The Complex Care Coordinator will have highly developed professional knowledge of complex discharge planning, service options and availability across the ACT and region, with the ability to lead multidisciplinary teams and provide education and supervision to a broad range of staff.
This role is required to participate in an after-hours roster, including regular weekend and public holiday work.

Canberra Hospital Expansion Project - CHS is constructing a new Critical Services Building called 'Building 5' which is a 44,000sqm nine storey building specifically designed to deliver state-of-the-art acute clinical services at the Canberra Hospital. North Canberra Hospital - As of 3 July 2023 Calvary Public Hospital Bruce transitioned to CHS and became North Canberra Hospital. The transition will deliver a health system networked under one provider and will provide increased workforce opportunities for staff at both CHS sites. The ACT Government will be building a new northside hospital on the existing hospital campus in Bruce to meet the growing health care needs of our community, with construction to commence mid-decade providing more beds, increased services, and increased career opportunities.
