The Connections Program works within an assertive linkage model that aims to link patients with relevant health and welfare service providers post release.The Connections Program team utilises a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs.Salary & BenefitsYour salary of $81,581 - $108,532 (+ super and leave loading) is just the beginning.
