Clinical Coordinator - Transitional Aged Care (TACP)
Service Delivery Directorate – Dubbo
About the role
To assess hospital inpatients approved for TACP by ACAT and develop a care plan which will deliver the care and clinical interventions they require to reach their restorative goals of independence. To coordinate services and monitor client progress within the multidisciplinary team and arrange and facilitate discharge from the TACP to appropriate long term care arrangements if required.
