A bit about the role
As a Continuity Liaison & Support Midwife for Wyndham City, you will join an integrated team which strives to achieve the best outcomes for Wyndham mothers and babies.
You will be part of a large multidisciplinary team of MCH nurses, immunisation nurses, child and family resource officers, lactation consultants, a social worker and a family violence officer, and well supported by a large leadership team. Applying your clinical skills and experience in a community focused environment, you will be responsible for facilitating a seamless continuum of care for clients from maternity, newborn and paediatric services into Council’s services.
Your highly developed interpersonal and communication skills will see you develop and maintain relationships and share information in accordance with legislative requirements with health, family and early years services to support access to Council’s service for all infants within the municipality, including those in Out of Home Care (OOHC).

What you’ll deliver
- Coordination of handover from hospital staff within Extended Postnatal Care; Special Care Nursery; Neonatal Intensive Care Nursery; and other relevant services regarding client needs post-discharge from hospital
- Liaison with Council service/s that will be providing care to the infant and family to ensure that information is provided in a timely manner to enable planning of care and minimize parental anxiety
- Partnering with other Community programs
What you’ll bring
- Current registration with AHPRA in General Nursing and Midwifery
- Extensive recent experience in domiciliary midwifery or special care nursery
- Demonstrated experience working with CALD communities
- Current valid driver’s licence and Working With Children check
- Confidence and competency in using computer systems
- A continuous improvement mindset with a positive, proactive, and flexible attitude
