About the Role
This position coordinates the delivery of services to older people and younger people with a disability living within the Shire of Esperance. The care is coordinated in partnership with the care recipient and primary carer and is based on a philosophy that encourages a person to remain living as independently as possible within their own home.
This position will work with Senior Coordination Officers, providing confidential support and assistance. The role reviews services and develops new care plans in consultation with care recipients and primary carers, ensuring care plans reflect their goals. There will be strong communication between various areas of Home Care, working in a team environment to ensure the delivery of service requests.
This position is part time, 30 hours per week, five days per week. Some after hours support via an after hours phone will be required.
About You
To be successful and thrive in this role, you will have:
- Experience within the aged care or disability services industry and a strong commitment to working with frail older people and younger people with a disability
- Qualifications in Aged Care and/or willingness to undertake Certificate IV in Ageing Support
- First Aid Certificate
- National Police Clearance and an ability to pass an NDIS Check
- Previous administrative experience and good computer skills
- Ability to work within a team environment

Salary: $34.44 per hour, plus superannuation
