East Pilbara Independence Support (EPIS) is currently recruiting for a full-timeSupport Coordinator and full-time Support Worker for our facility in Marble Bar.
About Us
EPIS has been delivering a wide range of out-patient aged care and support services to the most vulnerable members of Marble Bar, Newman, Tom Price and surrounding communities for over twenty years. These services include day centre support, home care, cottage respite and NDIS and are delivered by our multi-disciplinary team.
About Marble Bar
Marble Bar is a remote outback town located in the heart of the Pilbara region of Western Australia, with a population of approximately 200 people. Situated at a distance of approximately 200 kilometers southeast of Port Hedland, Marble Bar offers a unique blend of isolation and connection to the larger regional center.
Responsibilities of the roles include:
- Providing a range of community aged care services inclusive of personal care, domestic assistance, social support and transport
- Managing a Day Centre
- Support clients to access their community, engage with activities of their choice, attend appointments and access amenities
- Providing information and support to potential clients regarding aged care services available from EPIS
- Coordinating support services and developing care plans
What we are looking for:
- Prior experience as a Care/Support Coordinator in aged care
- Cert III or Cert IV in aged care/individual support required.
- Thorough understanding of contemporary aged care service delivery issues in rural and remote areas including Aboriginal communities
- Knowledge and understanding of HCP and CHSP
- Excellent communication and advanced relationship building skills
- WA Driver's license and ability to drive 4WD vehicles.
- Current and valid Police Clearance
- Ability to relocate to Marble Bar, WA
