An exciting opportunity has become available for a Customer Engagement & Liaison Officer (CELO) to join our team. The position is based in Horsham.
JOB PURPOSE
Customer Engagement & Liaison Officer (CELO) performs an integral role in business development, and Customer Engagement, ensuring individualised services & support plans are developed and delivered to, both new and recurring, customers including the arranging of short term accommodation needs of customers.
The CELO is responsible for the development of individualised support plans in consultation with customers, their family or advocates, key health professionals and other key stakeholders.
The CELO is responsible for liaising with the Rostering and Administration Assistants to coordinate the delivery of services to customers in line with service agreements and support plans and to provide quality oversight and reporting of the services back to key stakeholders.
MAIN DUTIES
Business development including prospecting for new customers, building rapport, grow and retain existing customers by presenting new solutions and services to customers.
Intake and assessment of NDIS/HCP/Private/Brokerage customers, including Home risk assessments, emergency plans, referrals to Registered Nurse for health checks, medication chart checks.
Undertake comprehensive customer assessment and planning by assessing, monitoring, reassessing and reviewing the needs of the customer
Complete service agreements, support and care plans and other legislative or JBC documentation.
Develop and manage customer service schedules, including service planning, confirmation and short notice placements and cancellations
Manage day to day activities of the customer journey.
Manage the Respite houses (short term accommodation – STA) booking calendar, service agreements, rostering of supports and property oversight such a supplies, cleaning and maintenance
Develop and Monitor the implementation of individualised plans for customer.
Develop strategies which will address the identified needs of the customer in consultation with their representative/s where appropriate, in accordance with funding guidelines.
Monitor customer budgets in accordance with program guidelines.
Provide feedback to key stakeholders including written reports.
Manage communication with key stakeholders – both internally and externally
Identify and participate in professional development
Ensure compliance with all Just Better Care policies and procedures, statutory and contractual requirements in relation to management of service delivery.
Be involved and available for on call rotation.
