Role
We are seeking to employ a Direct Care Coordinator (0.8 or 1.0 FTE) with an immediate start to be part of our enthusiastic and professional team. You will work in a collaborative and consultative manner to ensure our services not only meet but exceed our client's expectations.
The Direct Care Coordinator plays a critical part in the ongoing management and monitoring of individuals engaged in supports and services with Accept Disability. The role undertakes intakes, assessments and ensures that individuals have all the information and support they need to engage with Accept Disability services and to meet their individual ongoing goals. The Direct Care Coordinator will help with building an individual’s ability to exercise choice and control, to coordinate supports and access local community. They will also undertake tasks as per instructions given to them by the Management Team.
What the Responsibilities include:
- Completing in home assessments/intakes with participants and their families
- Developing support plans as directed by participants and/or their families
- Preparing progress and outcomes reports for plan reviews
- Providing budgets for service delivery with reference to participant funding
- Managing and resolving conflicts, incidents and complaints
- Assisting with implementation of plans and programs for individuals or groups to encourage and assist clients to achieve increased levels of independence
- Maintaining client records and documentation in a timely manner
- Liaising with other health and community care providers
- Attending networking meetings and develop good working relationships with other organisations in the sector
- Planning and implementing community events to market Accept
- Undertaking review and sign-up for existing participants at the start of their new NDIS plan
- Ongoing case management and care coordination
To be successful in this role, the following skills and experiences are sought:
- Tertiary qualification in a Certificate III or higher in Disability and/or Mental Health or equivalent or comparable working experience in the sector
- Excellent knowledge and understanding of the NDIS.
- Exceptional customer engagement and assessment skills.
- A willingness to work both autonomously and within a team.
- Demonstrated ability to engage and communicate with a range of individuals.
- Knowledge of relevant community services and the ability to establish partnerships with community services and agencies.
- Thorough report writing and attention to detail.
- Flexibility and reliability.
- Patience and understanding with a supportive and caring nature.
- Excellent verbal and written communication skills, with a professional and friendly demeanor.
- Proficiency in using customer relationship management (CRM) software and other relevant computer applications including Microsoft Word and Excel.
- Exceptional organizational skills and ability to meet deadlines.
- Strong problem-solving and conflict resolution abilities.
- Commitment to the rights of elderly and people living with disability to live dignified lives.
The following checks are also required:
- DHS Screenings for NDIS and Working with Children
- ‘Seeing through their eyes’ Child Safe training.
- National Police Check
- NDIS Worker Orientation Module ‘Quality, Safety and You’
- Current full SA driver’s licence and access to reliable and roadworthy car
- Manual Handling Certificate, CPR and First Aid certificate.
