- Supports families and provides education and care for disabled persons in adult service units, group housing and government institutions.
- Assesses clients' needs and plans, develops and implements educational, training and support programs.
- Interviews clients and assesses the nature and extent of difficulties.
- Monitors and reports on the progress of clients.
- Working with clients from diverse backgrounds to ensure their individual needs are supported while respecting their culture.
- Delivering services with compassion, integrity, and dignity.
- Following tasks as directed per Care Plans and Task lists, such as personal care, general housekeeping, social support, and transport.
- Provide 1:1 quality support and meet the requirements of the clients based on their individual needs and in line with their goals.
- Assist clients in accessing the community.
- Conduct client assessments to confirm services required, and clients' eligibility to receive them.
- Maintain accurate records and report writing on progress from the client and management reports.
- Provide feedback to the relevant Manager.
- Develop and employ strategies to attract clients to the disability care program.
- Liaising with community groups, welfare agencies, government bodies and private businesses and health care providers about community issues and promoting awareness of community resources and services.
- Prepare the program for audit in coordination with the Q&C Supervisor.
- Other duties as required by the Disability Care Coordinator.
SKILLS & EXPERIENCE:This position will require the following
- Relevant qualification in community services and at least 2 years work experience
