Role Overview:
The Disability Services Coordinator is a pivotal role focused on providing comprehensive support and guidance to individuals with disabilities and their caregivers. This role requires a dedicated professional who can conduct assessments, develop personalised plans, and collaborate with participants to achieve their personal goals. The successful candidate will also ensure that services align with program guidelines, maintain client confidentiality, and engage with the community to promote disability programs.
Key Responsibilities:
• Conduct holistic assessments and reviews for program participants.
• Develop and maintain individualised, flexible Care Plans.
• Ensure high-quality client services in compliance with regulations and policies.
• Collaborate with agencies and organisations to support participant goals.
• Engage in community networking to promote Disability programs.
Position Requirements:
• National Police Check & Working with Vulnerable People/Children Check (Employment registration NDIS endorsed)
• A valid unrestricted driver's licence.
• Minimum Certificate 4 in Community Services, Individual Support or equivalent
