Job Title: Disability Services Coordinator/Manager

Position Overview: We are seeking a highly motivated and compassionate individual to join our team as a Disability Services Coordinator/Manager. The successful candidate will play a key role in overseeing and coordinating disability support services, ensuring the delivery of high-quality care and support to individuals with disabilities.
Responsibilities
- Coordinate and manage all aspects of disability support services, including assessment, planning, implementation, and review of individual support plans.
- Develop and maintain strong relationships with clients, families, caregivers, and other stakeholders to ensure the delivery of person-centered care and support.
- Supervise and provide leadership to a team of support workers, ensuring they have the necessary resources, training, and support to deliver quality services.
- Monitor and evaluate service delivery to ensure compliance with relevant standards, regulations, and quality assurance frameworks.
- Conduct regular assessments and reviews of client needs and goals and collaborate with interdisciplinary teams to develop and adjust support plans accordingly.
- Advocate for the rights and interests of individuals with disabilities, promoting inclusion, dignity, and self-determination.
- Oversee the coordination of resources, equipment, and facilities to support the delivery of services and enhance the quality of life for clients.
- Maintain accurate and up-to-date documentation, records, and reporting systems in accordance with organizational policies and procedures.
- Participate in staff meetings, training sessions, and professional development activities to enhance knowledge and skills in disability support services.
- Collaborate with external agencies, community organizations, and government bodies to facilitate access to additional support services and resources for clients.
- Utilize rostering software or tools to streamline the rostering process, optimize staff scheduling, and maintain accurate records of staff availability, qualifications, and preferences.
- Create and manage staff rosters to ensure adequate coverage for all shifts, considering client needs, staff availability, and regulatory requirements.
Qualifications:
- Bachelor's degree in social work, disability studies, human services, or a related field (required).
- Minimum of 5 years of experience in the Disability Services sector, with demonstrated experience in service coordination, case management, or a similar role.
- Knowledge of disability legislation, standards, and best practices, including the National Disability Insurance Scheme (NDIS) (highly desirable).
- Strong interpersonal and communication skills, with the ability to build rapport, establish trust, and collaborate effectively with diverse stakeholders.
- Excellent organizational and time management skills, with the ability to prioritize tasks, meet deadlines, and manage competing demands.
- Demonstrated leadership abilities, with experience in supervising, coaching, and supporting staff in a team environment.
- Commitment to promoting the rights, dignity, and well-being of individuals with disabilities, and a passion for advocating for social justice and inclusion.
- Valid driver's license and access to a reliable vehicle (required).
