Mighty Mentors is looking for an additional team player who is motivated, passionate, highly skilled and NDIS experienced to join our dynamic team of Service Coordinators.
Service Coordinator role and purpose:
As a Service Coordinator with Mighty Mentors, you’ll be instrumental in guiding a team of mentors to ensure each individual participant receive high quality of support from suitably matched mentors, who will support participants to work towards achieving their personal goals, improve their everyday experience, giving participants choice and control and empowering individuals to live their best life.
Manage and oversee up to 55 NDIS participants, fully responsible of your allocated participants roster of supports, a line manager to the mentors who support the participants on your caseload. A Service Coordinators role is ensuring all Mighty Mentor participants receive high quality of services that are delivered in a person-centred approach, managing and overseeing quality and compliance ensuring your teams of mentors are following all the
correct processes, are up to date with policies and procedures, supporting mentors in their role to be equipped to deliver the highest standard of care and support to Mighty Mentors participants.
Responsibilities:
- First point of contact for your allocated participants and mentors
- Build and lead a team of mentors and overseeing mentors work performance is of high standard ensuring all policies and processes are correctly adhered to.
- All scheduling and rostering duties for your allocated caseload.
- Creating job cards for vacant shifts to advertise to mentors to apply.
- Mentor matching a suitable mentor with the right skill set to work with a participant.
- Onboarding new referrals allocated to your caseload.
- Face-to-face home visits, home risk assessments.
- Create comprehensive support plans.
- Complete comprehensive progress reports.
- Document all contact in participant’s profile ensuring all information is accurate and up to date.
- Ensuring mentors are completing progress notes for each shift and kms are kept within the allocated amount.
- Attention to detail and diligence in your own work practice ensuring all forms are completed correctly and always following the correct processes.
- Good time management.
- Building and maintaining strong rapport with participants, mentors, participants family and all stakeholders.
- Attend required stakeholder meetings.
- Ensure participants have enough allocated NDIS funding for the duration of their service agreement with Mighty Mentors.
- Ensure all service agreements, home risk assessments and support plans are kept current and up to date.
- Update all mentors with any changes for the participants ensuring all mentors know what their purpose is and what they are required to do on each support.
- Follow up on incidents and hazards ensuring all are followed up diligently, working towards the best outcomes ensuring all have been resolved and preventive measures are implemented.
- Conducting mentor supervision and work performance appraisals.
- Ability to adapt to changes in day-to-day work structure to allow for continuous improvements being implemented within the company.
- Strict practice of confidentiality.
- Ability to provide on call on a rotation basis within the team of Coordinators.
- Ability to work remotely within a fast pace, virtual team environment.
Skills, experience and attributes:
- Experience with NDIS and NDIS funding.
- Experience in case management and managing teams
- Motivation to provide service excellence to participants and leadership to mentors.
- Knowledge of NDIS services and supports.
- Experience with rostering.
- Excellent interpersonal communication skills.
- Demonstrated competency in excellent writing skills.
- Ability to develop, implement and oversee individualised support plans for each participant, ensuring participants needs are met and mentors deliver meaningful quality services for each participant.
- Knowledge and demonstrated ability with effective use of electronic document management systems, strong computer skills.
- Ability to problem solve effectively on a day-to-day basis.
- Ability to identify situations where the input of the General Manager is required.
- Ability to prioritise workload and demonstrated organisational skills.
- Ability to work autonomously, remotely and be part of a team in a virtual work environment.
