We are looking for team members in our supported independent living arrangements in multiple locations around Brisbane to provide 24/7 support in the home and out in the community.
The Support Worker's role and responsibilities will include:
- Supporting people with personal care and day-to-day activities in the home and out in the community
- Providing positive behaviour support to our clients
- Completing appropriate documentation through shift notes, incident reporting and data collection
- Contribute to the team’s overall objective of providing the best outcomes for our clients
- Work with your team leader to implement and review plans around daily service provision
- Maintain a professional relationship with clients and adhere to the NDIS and company code of conduct
- Above all, be responsive, reliable and accountable to our clients and your team

Qualification/Experience:
New to the industry desired!
Must have own form of Transport and a Driver's License
