Position Overview: We are looking for a dedicated Disability Support Worker who is passionate about assisting individuals with disabilities in achieving their employment goals and daily goals. The ideal candidate will have experience in providing person-centered support and possess the skills necessary to help clients develop vocational skills, secure employment opportunities, and thrive in the workplace.
Responsibilities:
- Provide individualized support to clients with disabilities to develop vocational skills and employment readiness.
- Conduct assessments to identify clients' strengths, interests, and areas for growth related to employment.
- Collaborate with clients to create personalized employment plans and goals.
- Assist clients in job searching, resume writing, interview preparation, and job application processes.
- Facilitate skill-building workshops and training sessions related to employment readiness, workplace communication, and job retention.
- Advocate for clients' rights and accessibility accommodations in the workplace.
Personal Care Assistance: Provide assistance with activities of daily living (ADLs) including bathing, dressing, toileting, grooming, and feeding, as needed.
Mobility Support: Assist individuals with mobility challenges in moving around their home or community, utilizing mobility aids or assistive devices when necessary.
Medication Management: Support clients in managing their medication schedule, administering medications as prescribed, and monitoring for any adverse reactions or side effects.
Emotional Support: Offer emotional support and companionship, actively listening to clients' concerns, providing encouragement, and engaging in meaningful conversations to promote mental well-being.
Community Integration: Facilitate opportunities for clients to participate in community activities, social events, and recreational outings, promoting social inclusion and connection.
Life Skills Development: Teach and reinforce skills related to independent living, such as cooking, cleaning, budgeting, time management, and problem-solving, to promote autonomy and self-sufficiency.
Advocacy: Advocate for clients' rights, preferences, and needs within various settings including healthcare facilities, educational institutions, and community services, ensuring they receive equitable access and treatment.
Behavioral Support: Implement strategies to manage challenging behaviors effectively, utilizing positive behavior support techniques and de-escalation methods to promote a safe and supportive environment for both the client and others.
Assessment and Planning: Conduct comprehensive assessments of clients' strengths, needs, and goals, collaborating with interdisciplinary teams to develop individualized support plans tailored to their unique circumstances and aspirations.
Documentation and Reporting: Maintain accurate and timely documentation of client interactions, progress notes, incident reports, and service plans, adhering to organizational protocols and regulatory requirements.
Crisis Intervention: Respond promptly and effectively to crisis situations, employing appropriate protocols and communication strategies to ensure the safety and well-being of the client and others involved.
Training and Education: Provide training and guidance to family members, caregivers, and other support personnel on best practices for assisting individuals with disabilities, promoting consistency and continuity of care.
Health Promotion: Promote healthy lifestyle choices and behaviors, including nutrition, exercise, and preventative healthcare measures, to support clients in achieving optimal physical health and well-being.
Collaboration and Networking: Collaborate with external service providers, community organizations, and healthcare professionals to coordinate care, access resources, and advocate for additional support services as needed.
Professional Development: Engage in ongoing training, education, and self-reflection to enhance professional knowledge, skills, and cultural competence, staying informed of emerging trends and best practices in the field of Disability Support.
Qualifications:
- Bachelor's degree in social work, psychology, disability studies, or a related field (preferred).
- Certificate III or above in Individual Support, Community Services or a related field (Essential)
- Previous experience working in Disability Support services, vocational rehabilitation, or a related field.
- Strong understanding of employment-related barriers faced by individuals with disabilities and knowledge of best practices in supported employment.
- Excellent communication, interpersonal, and organizational skills.
- Ability to develop rapport and establish trusting relationships with clients.
- Empathetic, patient, and compassionate attitude towards individuals with diverse abilities.
- Commitment to promoting independence, inclusion, and self-determination for clients.
- Proficiency in Microsoft Office and ability to maintain accurate documentation.
- Valid driver's license and access to reliable transportation (if required for job duties).
