We are currently seeking a Registered Nurse to join our Community Nursing team, this is a part time position of 20 hours per fortnight.

Community Nursing is a part of the broader Community Services Department which is located at the Ararat campus, we provide community nursing across the catchment area of Ararat Rural City. Community Nursing includes Palliative Care, District Nursing Hospital in the Home, Hospital Admission Risk Program (HARP), Home Care Packages and specialist nursing programs. Each program area aims to provide awareness and education which enables community members to have a better understanding of their own health issues and thus take greater responsibility for improving their own health outcomes.
What we OfferAbout the Role
District Nurses are independent practitioners who partner with Consumers, to create a person-centred design of care by including the consumer in a shared decision-making process to ensure clients are partners in their own care.
Responsibilities include assessment and care planning, facilitating and coordinating evidence based interventions relevant to the client’s condition(s), liaison with the team and other services.
About You- Current registration with AHPRA as a Registered Nurse
- A demonstrated commitment to the provision care that is client centered, strengths based and underpinned by a re-enablement and restorative philosophy
- A current and valid Victorian drivers license
- Ability to contribute and practice collaboratively as part of a multidisciplinary team
Appointment is subject to immunisation clearance (Including COVID-19) and current and satisfactory Employment Checks (Police and Working With Children Checks)
