Form of work The OpportunityWe are seeking capable and enthusiastic District Nursing Coordinator to join our team here at Benalla Health.The District Nursing Service’s major objective is to ensure high quality professional nursing care for people with short- or long-term health issues or disabilities, who choose to remain in their own homes. The District Nurse coordinator is involved coordinating home nursing service lists, workflow, and the need to see the consumers in their home, on in the consultancy room. KEY SELECTION CRITERIA:Experience in Acute Nursing, and an interest in Chronic disease self-management, consumer led goal directed care planning, person centred care and ability to work well within a team.Ability to attend to clinical needs for consumers in the consultancy room.Demonstrated ability to use electronic communication, software systems and relevant hardware.Demonstrated well-developed communication and interpersonal skills.Demonstrated ability to articulate clear decision-making processesAbility to attend one day a week, attending to HITH clients, wounds and other needs of the consumers.Current Car driving…
