About the Position
We are looking to welcome aFamily Support Practitionerto deliver an outreach model of family case-management programs to families, caregivers, kids, and young people within Intensive Family Support Team.
- Full time position.
- Level 4 under the SCHCADS Award. Salary range is $82,288 - $88,288 + Superannuation.
- Government funded until 30 September 2026 with possible extension.
Key Responsibilities:
- Maintain a regular case load of client families and regularly attend and participate in case reviews.
- Engage families, obtain consent, and develop case plans in conjunction with the family and which addresses unmet needs and child protection concerns.
- Working collaboratively with other professionals and agencies to ensure families goals are achieved.

About You
- A tertiary degree in a relevant discipline (allied health, social work, human services).
- Two years’ experience providing case-management and therapeutic services to families, caregivers, young people, and children in a child protection framework is preferred.
- New graduates, seeking development in case management skills is also an area of focus within the Intensive family Support program. Graduate applicants are encouraged to apply.
Other Requirements
- Eligibility for a Working with Children Check or professional exemption.
- Current driver’s licence.
- Legally able work in Australia (proof of eligibility will be requested).
- Fully vaccinated against COVID-19 or have an approved medical exemption.
