About Us?
The Institute for Urban Indigenous Health (IUIH) leads the development, planning and delivery of comprehensive primary health care for Aboriginal and Torres Strait Islander people living in South East Queensland.
Developed and implemented under First Nations leadership, The Birthing in Our Community (BiOC) program provides woman with access to dedicated Family Support Practitioners, case-load midwifery care, post-natal clinics, early learning programs and a social health team which includes social workers and psychologists.  The program is integrated within a family centred model and is supported by an Indigenous workforce strategy based out of a culturally-safe and welcoming Aboriginal Community Controlled community-based hub.
 
About the Role?

  • Our Family Support Practitioners are integrated within a family centered model and provides social and wellbeing support to women and family members throughout the woman’s pregnancy and up to three years following the birth.
  •  Create a pathway for culturally respectful engagement between women, midwives, and other service providers
  •  work effectively with other social and health practitioners to identify and access appropriate services and supports to meet their family’s needs.
  •  Manage a caseload of women ensuring regular contact and follow up to ensure priorities and action are met in conjunction with other team members (Midwife, Social Health Team, GPs)
  •  Use contemporary best practice behaviour change and relapse prevention models including Motivational Interviewing to increase ability, confidence, and readiness for change
  •  Assisting with administrative documents and navigating through complex social systems - completing housing applications, Centrelink forms, Birth registration
  •  Participate in the provision and review of effective quality Case Management – i.e. case reviews and case conferencing with all relevant parties.
  •  Working closely with women and her family to identify her social emotional wellbeing needs to ensure optimal outcomes for mother and baby.
  •  Identify risk that can impact on mother’s wellbeing, including but not limited to mental health, housing stability, domestic violence, child safety concerns, etc
  •  Work collaboratively with women to identify concerns and to develop a strength focused social wellbeing support plan that sets out priorities and action, including defining support/interventions from existing supports and local services
  •  Act as a central point of contact and access for women, particularly those with multiple complex presenting issues and providing interim support to acute/crisis presentation or while awaiting intervention from Social Health Team. 
 
About You?
Essentials:

  • Current Drivers Licence
  • Willingness to travel within the SEQ region
  • Criminal History Check or willingness to obtain
  • Valid QLD Blue Card  or willingness to obtain
  • Ability to work sensitively and effectively with Aboriginal and Torres Strait Islander communities, their leaders and health professionals, while respecting traditional culture, values, and ways of doing business.
  • Ability to promote the use of holistic strength-based approach that acknowledges the broader social and environmental influences on health and wellbeing
  • Previous experience working with vulnerable families, including working with children, youth and parents within the Indigenous community
  • Ability to effectively identify psychosocial factors influencing pregnancy and parenting and provide support and/or referral to specialist services.
  • Demonstrated experience in facilitating positive behaviour change with individuals, families and community groups.
  • Ability to manage a case load and work as part of a multidisciplinary team fostering a positive culture with excellent communication skills.
